What are the required documents families need to have ready to go upon registering?

Document Type Required

1 Birth Certificate per Angel

2 Food Stamp Print Out if applicable

3 Identification per Application Adult

4 Proof of Guardianship If Applicable

5 Proof of Income for Adults in Family (Pay Stub, SSI Benefits, Child Support, EBT, DCF Benefits, WIC, Unemployment Benefits)

6 Proof of Residency for Angel household

7 SNAP Letter with Amount and Names of Household Members if applicable

Notes:

· Please make sure to fill out all people living in the household. Please include teen children in the unqualified angel tree application portion.

· Include all income for all members in your household-income is any form of money received including WIC/food stamps.

· For your angels wishes please make sure they are toy related – no large electronics, all items should be between $15.00-$30.00 each. Also please give multiple options for each child so they have a better chance at receiving something they would like.

- Please note the Angel Tree program is for those in need of assistance and meet the required financial eligibility standards based on Wyoming State standards. Children 12 and under & seniors 65 and over.
